Caregiver Support Program

Family members are the primary providers of long-term care for loved ones.  Unfortunately, there is nothing simple about caregiving.  The Caregiver Support Program has been a part of Catholic Charities for over 29 years and serves 14 counties of the Diocese - Albany, Columbia, Delaware, Fulton, Greene, Herkimer, Montgomery, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Warren, and Washington.  The Caregivers Support Program is funded by the New York State Office for the Aging, Albany County Department for Aging, Rensselaer County Unified Family Services, the New York State Department of Health through a partnership with the Eddy Alzheimer’s Services, and private donations.

The Caregiver Support Program enables family members to maintain their quality of life and culture while caring for their loved one at home.   This is accomplished by providing support to caregivers to ensure they have the resources to meet the needs of caring for a loved one including support for them.

What Services Does the Program Provide?

Through 4 separate funding contracts, the program provides a spectrum of services.  Services available (vary by geographic area) include:

  • Care Consultation – trained staff meet with caregivers and care receivers to identify needs, develop a personalized plan aimed at meeting needs, as well as finding other resources in the community.
  • Support Groups – Caregiver support groups provide emotional support, information, offer caregivers the opportunity to share their experiences, and learn from others.  Support groups also serve as an opportunity for caregivers to remain engaged in their communities and to connect with others.
  • Respite Services – having the ability to access respite services, whether in their own home or in another setting, is often the most important service a caregiver can receive.  Respite care allows caregivers to take a break for themselves – for whatever reason is important to them – and know that their loved one is in trained, caring hands.

  • Telephone Support and Counseling – sometimes having someone to talk to over the telephone can be of great value to a caregiver.  Our trained professional staff are here to listen to your needs and offer compassionate support to your questions and concerns.

  • Information & Assistance – the wide range of services and options available to a caregiver can be overwhelming.  Our staff are specially trained to help you learn and understand what services are available and how to access them.  If our program is not able to help you, we will connect with a resource that can.

The Alzheimer's Caregiver Support Initiative seeks to alleviate the financial and emotional burden placed on caregivers.  Services are offered in a 10-county region (Albany, Columbia, Delaware, Fulton, Greene, Montgomery, Rensselaer, Saratoga, Schenectady, and Schoharie) and provide a wide range of free support and respite services for caregivers.

How Do I Access the Program’s Services?

Program access varies by county and service type:

  • Caregivers of individuals in any of the 14 counties of the Diocese who are 60 years of age or older and are caring for an individual with challenges other than Alzheimer’s or Dementia, may contact our main office at (518) 372-5667.  Staff will conduct an initial telephone screening, and as appropriate, schedule an in-home visit or provide you with the information needed to access other services, including Alzheimer’s and Dementia specific services.
  • Caregivers of individuals with Alzheimer’s or other Dementia will receive services through our Alzheimer’s Caregivers Support Initiative.  Caregivers living in Fulton, Montgomery, or Schoharie County should call our office in Amsterdam at (518) 369-5727.  Caregivers living in Columbia Greene, or Delaware County should contact our Catskill office at (518) 390-0341.  Following initial contact, program staff will schedule an in-home visit to fully assess your needs and make connections for necessary services.
